Admissions
Mathom House strives to be responsive to the needs of busy juvenile justice and social service professionals. Assistant Director, Mychal Harrison, is available to discuss your client’s treatment needs. Referrals can be initiated by telephone, fax, or email at mharrison@mathomhousecare.com.



Referrals are reviewed by the Clinical Director; in most cases, interview decisions are made within 24 hours. Interviews are scheduled at a time and location convenient to the client. A decision of acceptability will be made at the conclusion of that interview. On rare occasions the referral packet may need to be presented to the entire clinical team requiring that a final decision be delayed for 72 hours.

Referral Materials Should Include:
  • Psychological Evaluation (within 6 months)
  • Recent Psychiatric Evaluation (if applicable)
  • Current Social History
  • Medical and Dental Summary
  • Complete Academic Records (last grade completed)
  • Police Report (if available)
Admissions Procedures
For adolescents who are accepted, an admission date will be coordinated with all parties involved.


The Following Materials are Needed On/Before Admission Date:
  • A Court Order authorizing placement
  • Client’s current health insurance
  • Copies of the client’s most recent physical and immunization record
  • Copy of client’s birth certificate
  • Record of DNA collection
  • DHS Case Number (if applicable)
  • Managed Care Prior Authorization, if applicable
  • Copy of police report and victim impact statements

 

Mathom House provides services primarily to court mandated juvenile offenders. Other persons needing assistance with troubled youth should contact their local Mental Health and/or Children and Youth Services.
