Referral Procedure
The referral process can be initiated by phone, fax, or e-mail. Referrals are reviewed by the Clinical and Program Directors and in most cases we can review them and contact you to arrange an interview within 24 hours.
If your referral seems a likely candidate, Mathom House will request to interview the child at a time and location that is convenient to you. A decision of acceptability will be made at the conclusion of that interview. On rare occasion the referral packet may need to be presented to the entire clinical team requiring that a final decision be delayed for 72 hours.
SUPPORT MATERIAL SHOULD INCLUDE:
- Most recent (within 6 months) Psychological Evaluation
- Current Social History
- Recent Psychiatric Evaluation if pertinent
- Medical, dental summary and immunization records
- Complete school records indicating last grade child successfully completed
- Police reports if available

Admissions Procedure
For adolescents who are accepted, an admission date will be coordinated with all parties involved.
THE FOLLOWING MATERIALS ARE NEEDED ON OR BEFORE ADMISSION DATE:
- Court order
- Current health insurance
- Copy of most recent physical examination
- Copy of birth certificate
- Copy of victims impact statement
- Record of DNA testing
Mathom House welcomes, and strongly encourages, parents/guardians to attend on the day of admission to sign necessary paperwork and meet Mathom House staff.
